Comfort, Care & Followup

Whenever possible, we strive to make our patients as comfortable as possible. From a comfortable facility and courteous staff, to the best care and follow up. We provide all the resources needed, whether it is surgery, medication, clinical support, or patient education, for anyone affected by glaucoma.

Scheduling Appointments

To schedule an appointment with any of our physicians please give our office a call at (518)-475-7300. Our office hours are Monday-Friday 8:00-4:30.

Once you make an appointment with one of our receptionists we will send you a Welcome Packet that includes a map with directions to our office, our Practice Brochure and the information sheets that need to be filled out. We ask that you please fill them out prior to your visit with us as this facilitates your check-in. We also ask that you bring your insurance card(s) and referral (if necessary). Please feel free to give us a call if you have any questions.

Insurance

Glaucoma Consultants participates with many insurance companies including Medicare, Capital District Physicians Health Plan, Empire Blue Cross/Blue Shield, MVP, Aetna and many more. When you make your appointment with us we will ask you what your health insurance company is. While we participate with most insurance companies each individual plan is different. It is your responsibility to check with your insurance company to make sure we are a participating provider.

If you have an insurance plan that requires you to have a referral before seeing a specialist, the referral must be presented when you check in. If you are unsure whether a referral is required, please contact your insurance company. You are financially responsible for any services not covered by your insurance company.
